LIFE Senior Care, a program designed to assist seniors with their customized healthcare needs, located in Jersey City Medical Center, is currently seeking a Registered Nurse. This full-time position offers competitive compensation, a robust benefits package and generous paid time off.
The incumbent will perform various nursing related tasks that include assisting with participant assessments in the clinic or in the home and/or following an incident or during the performance of daily assigned duties and assists with implementation of the plan of care coordinated by interdisciplinary team.
Duties include the following:

Assists with participant assessments in the clinic or in the home and/or following an incident or during the performance of daily assigned duties.
Assists with implementation of the plan of care coordinated by interdisciplinary team.
Administer and/or supervise the appropriate dispensing of participant medications excluding intravenous push medications. Documents and tracks medications per policy.
Pre-fills medication boxes in the home per physician orders and schedule as needed.
Counts narcotic daily.
Document per SOAP note style all patient encounters.
Maintains vaccine/medication refrigerators. Administer vaccinations for participants and team members.
Reconciles participant medication record with all medications and treatments ordered by provider.
Perform wound measurements and document findings on a weekly basis.
Assist with phlebotomy.
Transcribe provider orders into the medication record.
Perform monthly clinic environmental rounds.
Appropriately communicates participant condition with LIFE staff; assisting with supervision of the Clinic CNAs and Medical Assistants/Home Health Aides.
Completes annual compliance and HIPAA training and exhibits behavior as set forth by the LSMNJ Code of Ethics and Business Integrity in the performance of duties.
Adheres to the Organization’s Policies and Procedures regarding the Compliance Program of the organization and is timely in reporting any possible concerns.
Follows all local, state and federal regulations as they pertain to the position.
Adheres to the Organization’s Policies and Procedures covering Privacy and Security in Compliance with HIPAA regulations.
Other duties as assigned by supervisor, necessary for the efficient operation of the department/facility.
