Job Description

Positions can be in North and Central NJ

  1. Assists consumer(s) to implement Individual Service Plan goals
  2. Provides companionship and individual supports to consumers participating in recreational, leisure, and social activities
  3. Assists in adapting activities and materials according to individual needs
  4. Provides one-on-one tutorial assistance as needed
  5. Accompanies the consumer to community activities of his/her choice
  6. Assists consumer(s) with daily needs which may include transportation, transferring, toileting, grooming and feedin
  7. May implement behavior interventions with support from behavior specialist
  8. Follows through on program details as required
  9. May collect and record data
  10. May produce progress notes, periodic reports or other forms of recordkeeping
  11. Responds appropriately to input and direction from other team members
  12. Demonstrates initiative.8. Attends and participates in staff meetings, individual supervision meetings and trainings as assigned; may be required to attend off-site trainings, staff meetings and conferences
  13. Performs other functions as directed by supervisor, CPNJ management, or funding source representatives.

What is a Talent Network?

QUALIFICATIONS:

High school graduate; at least one year’s experience in a non-profit service delivery organization or an equivalent combination of education and experience; demonstrated ability to engage in strenuous physical activity; fingerprint examination and approval; valid driver’s license; successful TB screening.

Valid New Jersey Driver’s license (Standard) - Required upon hire

Pass CPNJ Driver Training & Testing

4 points or less against driver’s license

Prior to working with individuals and/or Prior to implementation of a behavior plan

Positive Behavior Supports & Functional assessment (Via the Boggs Center)

Required Within the First 2 weeks of Employment:

o Preventing Slips, Trips, and Falls

o Bloodborne Pathogens

Required Within the First 30 Days of Employment:

Life Threatening Emergencies (Danielle’s Law)

Required Within the First 90 Days of Employment

2-day New Employee Orientation Program

Cardio Pulmonary Resuscitation (CPR)

o Must be recertified every 2 years

First Aid

o Must be recertified every 2 years

AED Training

o Must be recertified every 2 years

Elwyn Crisis Management Training

o Must be refreshed annually

Medication- College of Direct Support (CDS) Transcript

o Medication Practicum-15 Days of completing the Medication Certification

Shifting Expectation: Changes in Perception, Life Experience, & Services Certificate (College of Direct Support (CDS) Transcript
